Guidelines for completing the Participation Agreement
• Organisations should only complete the designated signature fields and must not modify the Participation Agreement
• Digital signatures can be used to sign the Participation Agreement
• The Participation Agreement must be signed by an authorised representative from the organisation
• Save your Participation Agreement using the following name:
DVS - Participation Agreement - [Insert Organisation Name] - [Insert ABN/NZBN].
Instructions on how to upload the signed Participation Agreement to Noggin
• Save your Participation Agreement in the PDF format with the following name: DVS - Participation Agreement - [Insert Organisation Name] - [Insert ABN/NZBN]
• Click on the Noggin platform public form here which will direct you to the Participation Agreement Submission Form
• Complete all required fields in the form, including entering the mandatory Participation Agreement ID provided to the user in the email from IVS.Manager@ivs.nogginapp.io
• Use the ‘Upload’ option to attach your signed Participation Agreement and submit the form for processing.
Once you submit the form, you will receive a confirmation email from the IVS Manager advising that the Participation Agreement has been successfully submitted for consideration. The IVS Manager will contact you if further information is required. A subsequent email will be sent to you once the agreement has been approved.
